Caregiver Benefits

Participant Care Services
- Live with a caregiver who provides assistance with personal care and medical needs.
- Receive a personalized care plan with continuous support from a nurse and social worker.
Eligibility Requirements
- Must be able to meet the participant’s daily care needs and adhere to program guidelines.
- Can be a family member or friend, excluding a spouse or legal guardian.
- Must reside in the same home as the participant.
Eligibility for Participants Needing Care
- Must be 22 years or older.
- Unable to live independently.
- Have a chronic medical or psychiatric condition.
- Require assistance with at least one activity: bathing, dressing, toileting, transferring, ambulating, or eating.
- Covered by MassHealth Standard or CommonHealth, or may be a member of a Senior Care Options (SCO), OneCare plan, or PACE program.

We provide a committed care team to assist members with booking appointments, arranging transportation, managing medication refills, and connecting with community programs to maintain and improve health. Our team offers education on chronic conditions in the member’s preferred language, eliminating language barriers and ensuring clear, personalized guidance.
Holistic Mind & Body Care
We focus on educating members and caregivers about mental health and teaching simple mindfulness techniques, such as meditation, to help reduce anxiety and depression. With over 14 years of community care experience, our director develops a personalized care plan for each member. Understanding the challenges of aging minds, our approach emphasizes “small moments, repeated often,” ensuring patience, consistency, and unwavering support for our seniors.
Exclusive AFC Care Coordination
We are the only AFC agency that fully manages the administrative and care coordination responsibilities of an AFC caregiver, giving you more quality time to spend with your loved ones.
Empowering Seniors Through Technology
We believe technology can solve many challenges in senior care. Every member enrolled in our program receives a tablet with unlimited Wi-Fi, enabling telehealth appointments, seamless interaction with our care team, and participation in group classes—all from the comfort of home.
Member Rewards Program
Members who complete the “Healthy Habits” tasks during their first year of enrollment receive $100 to spend or can choose a “Date Night”—an all-expenses-paid evening for the senior and a guest.

We are available around the clock for our members via chat or call during office hours for immediate assistance. Calls outside office hours are answered by a monitored chat system, with non-urgent matters returned within 12 hours. For life-threatening emergencies, members should call 911 immediately.
